Disenrollment Request

If you are planning on disenrolling from our school and enrolling in a school outside of our district, please follow the link below and complete the attached form.  This will automatically notify us of your intent to disenroll and someone from the office will be in touch with you ASAP.  Completing this form does not disenroll you from school, it only starts the process.

 

 

Note, this form is not intended for transfer to another school in our district (ie Hawthorne, Lawndale, Leuzinger, or Lloyde).  If you are interested in changing schools within the district, please contact your academic counselor for next steps.
